Trillium Health Resources is receiving a $1.6 million allocation from a Substance Abuse and Mental Health Services Administration grant that the North Carolina Department of Health and Human Services Division of Child and Family Well-Being has been awarded. 

This allocation will expand High Fidelity Wraparound within the region, support a dedicated HFW liaison at Trillium, add two new HFW teams, and provide support for an existing HFW team within the region.

HFW assists youth and their families to achieve the changes they want in their lives. 

The interactive, team-based approach employs 10 principles and four phases to help families reach their desired outcomes and develop the skills needed for the future. 

The support teams in HFW include a coach, up to four wraparound facilitators, and three support partners. 

The youth served by HFW have multi-system involvement and complicated challenges. 

HFW seeks to intervene before these challenges can lead to the use of restrictive levels of care, high costs, and poor outcomes.

“Trillium welcomes this additional funding so that we can support even more people with HFW,” shared Leah Smith, innovative development director at Trillium. “In the past year, more than 35 families have neared completion of HFW services. They have reported increased communication, learned about community resources, and decreased their need for crisis responses due to plans put in place.”

Smith said, “This funding will increase access in our area to collaborative HFW teams that partner with youth and families to develop and achieve their own unique family visions, with the support that youth and families want and need.”
