Care Coordination for at Risk Children (CMARC) Public Health

Description: This position provides case management for Medicaid children, ages birth through 5, who meet certain risk criteria for enrollment in the Care Coordination for at Risk Children (CMARC) Program. The main goals of CMARC are to improve health outcomes and reduce Medicaid costs for enrolled children. The case manager works with families to identify and access community resources to meet their needs and encourages and supports families in the development of skills needed for effective problem solving, making decisions, and enhancing the capacity of families to meet the special needs of their children. Other duties include assessing, planning, counseling, evaluating, and reviewing records of clients in this program; and performing related duties as required.

Requirements: Bachelor's degree in social work from an appropriately accredited institution; OR bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; OR bachelor's degree from an appropriately accredited institution and two years directly related experience; OR an equivalent combination of education and experience. Preferred: Master’s degree or Bachelor’s degree in Social Work. All health department employees (with the exception of individuals with an accommodation or medical exemption) are required to get an annual flu shot vaccine as part of condition of employment.

Compensation: The hiring range for this position is $ 36,298 - $ 45,960. Halifax County provides a comprehensive employee benefits package, including membership in the N.C. Local Government Employees’ Retirement System. 

Signing and Retention Bonus: A non-refundable signing bonus in the amount of $3,750 will be paid in the selected applicant’s first paycheck following employment and execution of the Employment Signing and Retention Bonus Agreement. A retention bonus in the amount of $3,750 will be paid in the selected applicant’s next paycheck following the completion of 12 full months of employment in the same position with the County following the date of payment of the signing bonus.
