Details of the false claims and supporting false claims in the Roanoke Valley Rescue Squad lawsuit are provided for 10 representative patients but their names and personal information were deleted to comply with Health Insurance Portability and Accountability Act standards and protect patient privacy.

 

Patient 1

 

September 8, 2004: RVRS noted patient was unable to get up from bed without assistance and was unable to ambulate, sit in a chair or wheelchair and was bed confined.

Patient 1 was able to use a wheelchair and was not bed confined.

 

Patient 2

 

June 9, 2005: RVRS noted patient was bed confined and unable to stand or apply weight.

Patient 2 used a walker and was not bed confined.

 

Patient 3

 

October 22, 2005: RVRS noted patient was unable to support self or apply weight to legs and was unable to stand.

Patient 3 typically walked to the front of her house to get on the stretcher for transport.

 

Patient 4

 

September 28, 2004: RVRS noted patient was unable to get up from bed without assistance, unable to ambulate and unable to sit in a chair or wheelchair and was bed confined.

The records provided by RVRS do not establish Patient 4 was unable to sit or the medical necessity for ambulance transport.

 

Patient 5

 

May 27, 2004: RVRS noted patient was unable to get up from bed without assistance, unable to ambulate, and unable to sit in a chair or wheelchair.

The records provided by RVRS do not establish Patient 5 was unable to sit or stand.

 

Patient 6

 

October 27, 2004: RVRS noted patient was bed confined and unable to stand.

The records provided by RVRS do not establish Patient 6 was unable to sit or stand.

 

Patient 7

 

May 14, 2004: RVRS noted patient was unable to get out of bed without assistance, unable to ambulate, unable to sit in a chair or wheelchair and was bed confined.

The records provided by RVRS do not establish Patient 7 was unable to sit or stand.

 

Patient 8

 

March 23, 2004: RVRS noted patient was unable to get up from bed without assistance, unable to ambulate and unable to sit in chair or wheelchair and was bed confined.

The patient was placed in a chair at the dialysis center.

 

Patient 9

 

February 25, 2005: RVRS noted patient was unable to get out of bed without assistance, unable to ambulate and unable to to sit in a chair or wheelchair.

The records provided by RVRS do not establish Patient 9 was unable to sit or stand.

 

Patient 10

 

February of 2006 to August of 2008: RVRS noted patient was provided ambulance transport three times a week because patient was unable to stand or apply weight to his legs.

Patient 10 was observed walking to the ambulance with his wife’s assistance  and then climbing onto an RVRS stretcher on August 9, 2007, and Patient 10 reported he is able to walk short distances and that he has been transported by RVRS three times a week since February of 2006.
